Special Supporter of National MOTTEP and its 20th Anniversary
Supported by initial funding from the National Center on Minority Health and Health Disparities (NCMHD), National MOTTEP began and admininistered the MOTTEP Telehealth Center.
The Center included two programs - the Telehealth Hypertension Program and a Telehealth Diabetes Program. Both of these programs promoted patient self-management by providing patients with the appropriate tools, disease monitoring, e-education and video conferencing with a nurse manager.
While ultimately, the programs were similar, the Hypertension Program utilized centrally located kiosks or health work stations and the Diabetes Program utilized in-home laptops and other peripherals provided by National MOTTEP.
MOTTEP TELEHEALTH CENTER GOALS:
* To seek more effective ways to help patients self-manage chronic disease;
* To expand access to health management services;
* To provide regular feedback to patients on their individual health conditions;
* To raise awareness and knowledge on causes of chronic disases such as
hypertension and diabetes.
With more than 25 million Americans having diabetes and more than 26 million suffering with chronic kidney disease, National MOTTEP must continue to address these issues if its mission is to be effectively addressed - decreasing the rate and number of ethnic minority Americans needing organ and tissue transplants.
